NGAL，也稱為致癌基因24p3或脂質(zhì)運(yùn)載蛋白-2（LCN2）通過螯合鐵參與先天免疫，進(jìn)而限制細(xì)菌生長(zhǎng)。它在嗜中性粒細(xì)胞中表達(dá)，在腎臟，前列腺和呼吸道和消化道的上皮細(xì)胞中表達(dá)水平較低。
在急性腎損傷（AKI）的情況下，NGAL在受傷后2小時(shí)內(nèi)以高水平分泌到血液和尿液中。因?yàn)镹GAL具有蛋白酶抗性且很小，所以蛋白質(zhì)很容易在尿液中排泄和檢測(cè)。AKI患者的NGAL水平與其預(yù)后的嚴(yán)重程度相關(guān)，可用作AKI的生物標(biāo)志物。NGAL還可用作慢性腎病，造影劑腎病和腎移植等手術(shù)的早期診斷。
腎臟健康常見的是血清肌酐。血清肌酐是腎功能的標(biāo)志物，而NGAL是腎損傷的標(biāo)志物。NGAL水平是診斷AKI的更和敏感的標(biāo)志物，而不是血清肌酐水平。事實(shí)上，已經(jīng)證實(shí)NGAL尿排泄的增加是由于在通過其他方法檢測(cè)到任何損傷之前發(fā)生的管狀改變。因此，監(jiān)測(cè)NGAL水平可減少延遲的AKI診斷和治療。使用更敏感和特異的標(biāo)記物可以更早地診斷，正確應(yīng)對(duì)AKI，降低發(fā)病率和死亡率。
